Home is where we tend to feel the most comfortable. It’s where we can take off our shoes, put up our feet and relax. And it’s the place we prefer to be when we don’t feel well.

  • Were sent home from the hospital and are not fully ready to be left alone
  • Have had surgery and a dressing must be changed regularly
  • Require therapy but can’t get to outpatient appointments
  • Were recently put on a lot of new medications and are having trouble managing them
  • Need help with bathing and preparing meals
  • Are a diabetic with a foot problem and are unable to get to a doctor

In each of these situations, you simply need some assistance, often for a short time. And just a little help now can often keep a health problem from becoming worse.

Home health care is designed to provide you with the medical care you need in the comfort of your own home. If you or someone you know need medical care, and find it difficult to leave the house, or cannot even walk without help, our professional staff can come to you.

Sterling Home Health Care is a nationally recognized health care agency that provides home care for patients who qualify for our services. Although we assist some younger patients, most of our patients are age 65 and older. They range from patients who are normally independent and need short-term care, to those who have a serious medical condition and require intensive and/or long-term assistance.

Arranging for Home Health Care

Sterling Home Health Care is Medicare certified and Community Health Accreditation Program (CHAP) accredited. CHAP accreditation means we’ve met standards of excellence in delivering the best care and services possible for Medicare-certified home health and hospice providers.

We offer a full range of skilled and non-skilled home health care services based on individual needs. The primary home care services we provide are:

  • Skilled nursing
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Home health aides
  • Medical social work

We can arrange for professional medical personnel to visit your home once or twice a week, or more frequently if needed. We have nurses who can perform many of the tasks that would normally be done in a medical office, or even a hospital. There are therapists who can help you increase your mobility and make everyday life easier. And home health aides can help with bathing and fixing meals.

Your doctor can give you a referral for our services, or you can contact us yourself and we’ll get in touch with your doctor. It is your doctor who will decide which services you will benefit from most and write the orders for them. Typically, a nurse will be your case manager and coordinate the services you need. Your case manager can also offer other resources that you may find useful. For example, we can direct you to a physician who makes house calls, or even a visiting foot doctor, if you have that need.
